Photo Consent Form for Minors
Dear Parent/Guardian,
We are writing to request your permission to take photographs and/or video recordings of your child, during activities and events organized by Next Level Strength & Fitness. These photographs and recordings may be used for promotional materials, including but not limited to:
• Print and digital publications
• Social media platforms
• Website content
• Advertising and marketing materials
By signing this form, you grant Next Level Strength & Fitness the right to use, publish, and distribute images and/or video recordings of your child in various media formats. You understand that these images may be used for educational, promotional, or marketing purposes and that Next Level Strength & Fitness will not sell these images for commercial purposes.
Additional Terms:
• I understand that my child’s name or personal identifying information will not be used in conjunction with any images without additional consent.
• I reserve the right to withdraw this consent at any time by providing written notice to Next Level Strength & Fitness. Withdrawal of consent will not affect any images that have already been used or published prior to the withdrawal.
• I release Next Level Strength & Fitness and its representatives from any claims, demands, or liabilities that may arise in relation to the use of these photographs and/or video recordings.
